Please be sure to bring a current insurance card at the time of your scheduled appointment to avoid full payment for the services rendered.
Out-of-Network Insurance
Out-of-network insurances are usually discussed on a case by case basis. If you wish to pay in cash for the services rendered, you will receive a courtesy discount. Please feel free to contact my office, so that we can better assist you.
Co-payments
Please, we aware that any balances, co-payments and/or co-insurance are expected on the day of service unless other payment arrangements have been made. For additional information, you can look at our Financial policy listed under "Patient Forms."
